Since its inception in 2020, the Community Care HUB has been meeting participants where they are at to improve their health and well-being. The HUB follows the nationally recognized and evidence-based Pathways Community HUB Institute (PCHI) model developed by Drs. Sarah and Mark Redding.
Face-to-face visiting between Community Health Workers (CHWs) and participants is a top priority. Six CHWs are employed by HUB Care Coordination Agencies – Alleghenies United Cerebral Palsy, Beginnings, Inc., and Community Action Partnership of Cambria County.
The PCH model utilizes a whole-person approach where 21 Standardized Pathways have been designed to address participant needs.
The 21 Pathways CHWs used are:

- Family Planning
- Food Security
- Health Coverage
- Housing
- Immunization Referral
- Learning (48 evidence-based learning modules)
- Medical Home
- Medical Referral (25 types of Medical Referrals)
- Medication Adherence
- Medication Reconciliation
- Medication Screening
- Mental Health
- Oral Health
- Postpartum
- Pregnancy
- Social Service Referral (28 Types of Social Service Referrals)
- Substance Use
- Transportation
Once the participant and the CHW identify a need, a corresponding Pathway is opened. Specific steps need followed to close a Pathway successfully as complete. For example, if Employment is identified as a need, the participant will have to obtain employment and remain employed for 30 days in order to close the Pathway as complete.
The PCH model is unique because the CHW is not just providing the participant with information on resources related to the Pathway or need, but is working together with the participant to connect them to the resource and ensure the need is met.
