One of the major initiatives of the CPH is to address Social Determinant of Health Needs in our community, which includes food education and security. In 2020, community leaders participated in a Local Foods Local Places Technical Assistance program facilitated by the U.S. Environmental Protection Agency (EPA), U.S. Department of Agriculture (USDA), and project partners. The workshop focused on building and strengthening institutions and partnerships to advance our local food systems, which include some of the following initiatives:
- Developing a resource inventory of food related resources and educational materials
- Expanding nutrition and education
- Addressing food insecurity
- Using food as Economic Driver
- Expanding community gardens
- Promoting and expanding Farmer’s Markets
- Exploring grants and best practice opportunities

Self-Referral Form
Below are two links to referral forms for food-related resources. Both links are online referral forms that are sent to the Greater Pittsburgh Community Food Bank (GPCFB) or the Central Pennsylvania Food Bank (CPFB). The links will not sign you up for SNAP benefits. Someone will contact you from the Food Bank to complete the paperwork.
If you live in Cambria, Indiana, Clearfield, or Somerset County use the Pittsburgh link. If you live in Blair, Bedford, or Centre County use the Central Pennsylvania link.
