1889 Jefferson Center for Population Health announces Request for Proposals seeking Pathways Community HUB Care Coordination Agency partners in Cambria and Somerset counties

JOHNSTOWN – The 1889 Jefferson Center for Population Health, established through a partnership between the 1889 Foundation and the Jefferson College of Population Health, has released a Request for Proposals (RFP) seeking local partners to serve as Care Coordination Agencies under the Pathways Community HUB model (HUB).

The HUB helps to coordinate community resources to reduce the medical, behavioral and social barriers to care, such as employment, housing and transportation, for individuals with complex health needs.

A HUB is an organized, outcome focused, network of Care Coordination Agencies (CCAs) who hire and train community health workers (CHWs) to reach out to those at greatest risk. Community agencies are eligible to respond to the RFP if they provide, or are willing to provide, home-based outreach services to residents of Cambria and Somerset Counties and the organization has experience working in underserved communities of Cambria and Somerset Counties.

Information sessions were held last month in Johnstown and Ebensburg for agencies who were interested in learning more about this partnership opportunity.  The Pathways Community HUB plans to be operational by late spring.

Applications are due by January 14, 2020 by 12:00 pm (noon) and may be mailed or emailed (in PDF format) to:
