The Community Care HUB, a major initiative of Center for Population Health (CPH), was recertified through Pathways Community HUB Institute® (PCHI®).
To obtain recertification, the Community Care HUB met the stringent national standards for quality community care coordination services and is dedicated to excellence. To become certified for three years, it means the HUB went through a rigorous peer review process.
“We are honored to earn recertification,” said CPH Associate Director Kimberly Roberts. “This achievement shows our commitment to providing high quality services and our adherence to the fidelity of the model. Recertification demonstrates our continued commitment to creating health equity in the community.”
The HUB is certified effective November 12, 2024, through November 9, 2027.
The HUB was established in September of 2020 and assists individuals in Cambria and Somerset counties.
A HUB is an organized, outcome-focused network of Care Coordination Agencies (CCAs) who hire and train Community Health Workers (CHWs). The CHWs meet with HUB participants face-to-face, preferably in their own homes, and guide them through a series of Pathways to address their needs. CHWs also connect participants to need services.
Founded in 2015, PCHI is the nation’s only certifying body for Pathways Community HUBs.
