The Women’s Help Center has joined the Community Care HUB as a Care Coordination Agency (CCA).
The HUB, an initiative of Center for Population Health, aims to improve health outcomes of vulnerable populations by addressing risk factors, or social determinants of health, associated with poor health outcomes.
“We are thrilled to expand our program and partner with the Women’s Help Center. Their understanding of the needs of our community will help us connect many more at-risk people to resources and support.” said Center for Population Health Associate Director Kimberly Roberts. “For years the Women’s Help Center has proven to be a crucial part of the social service network and forging a partnership allows us to further our collective goals to improve health and wellness.”
By becoming a CCA, The Women’s Help Center will employ two Community Health Workers (CHWs). Once trained, the CHWs will meet with referred participants one-on-one, preferably in their homes, and connect them with needed services such as employment, housing, transportation, and food.
“We have been looking for additional ways to provide enhanced support to the individuals we serve in our emergency shelter and housing programs,” said The Women’s Help Center Executive Director Roxanne Tyger. “The Community Health Workers will add a deeper dimension to the work we do and help at-risk individuals and families be successful, healthy, and supported in the community. They will be a tremendous addition to our team.”
HUB referrals can be made by medical providers, social service agencies and community-based organizations. The HUB even accepts self-referrals.

The Community Care HUB launched in 2020 and follows the nationally recognized and evidence-based Pathways Community HUB model developed by the Pathways Community HUB Institute.
